My first thought is just like open notes on bass in GH:WT, but then what about an open note with one or two notes on top of it?
Impossibe for guitar/bass. you cannot have an open bass "sixth fret" and one of the five main frets as a chord on a guitar or bass. this is like a divide by zero sort of thing: you just don't do it just because the game controler is set up a certain way. (now, if you had TWO strum switches on the same controller, that's a different story!)
random **** cuz i was hungry:
If you are wondering about extended sustains for guitar and bass then I might have your answer.
If our so called "sixth fret" (also known as the open bass note, first introduced in GH:WT for bass guitar)... eh.. i'll keep it simple... (too damn hungry...)
If you think about it, an open strum is just a percusive "strum" on a string. Treat it like a drum note as it were. I've played through all of the GH:WT songs on GH:WT, and I have yet to have seen the "sixth fret" become a sustain. It has always been more of a percusive sort of thing.
An extended sustain with our elusive "sixth fret" would look like this.
(were a line of 0, is your open strum dashes are white space)(damn... i wish we had unifont...)

Technically, the open strum is still "sounds", but it is scored as a single note that is not sustained.
which brings me to something off topic: guitar body knocks. they need to implement a sensor so you can be scored for guitar body knocks.
