Today, Chris Cornell (Soundgarden and Audioslave's frontman) passed away at age 52, after playing a show with Soundgarden. The cause of death is thinked of being suicide.

You CANNOT use a regular mp3, flac, wav.. They will NOT work. you need an single instrument on its own track for auto note placement apps to work and even then they dont get half the notes correct. The midi files are not complete/corrupt. Look, MIDI files have are divided in instruments (parts), each instrument has an specific name to work with the game, in case of the guitar it's "PART GUITAR", that instrument of the midi named "PART GUITAR" contains the notes of the song ...

Not sure but I think it might be something with your sound card or the drivers, try updating them, it might be also the speakers/headphones you are using. If it keeps happening try re-installing the game (make a backup of your songs if you want).

Most of those games have those songs since the bands influented or are influenced by the band that the game is about. Also, adding additional songs by other bands is better, or you will have only songs from one band and that would be kind of boring
