I use a good ol' Black SG from GH1. I like the clicky strum bar, and not-so-clicky frets. I use a clear-blue PS2-to-USB adapter, its a 2-port adapter and it's shaped like a wedge with rounded corners. It doesn't recognize the whammy bar, though. Everything else works great, though. I just downloaded Alarian mod last night, and I'm thoroughly enjoying it. It feels very polished. I'm kinda bummed that my PS2pad adapter doesn't support the whammy bar, but I'll bet I could hack together a Star-power stomp-box with my Wii Balance board or DDR dance pad (Wouldn't THAT be awesome...)...
